Routine appointments can be booked up to six weeks in advance. Appointments can be made online using our website - registration is easy at any of our surgeries.

Bookings are also taken daily by telephone 01422 415777 from 8.00am - 6.30pm or you can call in to the surgeries at Hebden Bridge and Mytholmroyd to make an appointment. If phoning, our receptionists will ask you a few simple questions to ensure the most appropriate Clinician sees you.
Home Visits
If you are physically unable to get to the surgery, please telephone before 11.00am, Monday – Friday, with full details of the problem so that we can judge the urgency of your call. The Practice covers a large rural area and therefore home visits require a lot of travelling time. Please come to the surgery if at all possible. If you are not sure whether you need a home visit, please telephone and ask for advice.
Telephone Consultation
If you require telephone advice, please call the surgery. The receptionist will offer you the next available appointment and take some details of your request. The Clinician will return your call on the date and time booked. The Clinician will view your medical record before and during the consultation and is able to prescribe where appropriate. We can call patients on mobile phones but prefer to use a landline. Please note that for reasons of confidentiality, we are unable to leave messages on answering machines.
The practice records all telephone calls. These recordings are strictly confidential.

Self Certification Forms
We no longer supply SC2 self certification forms. There is a website where you can access the SC2 to print yourself. Please visit www.hmrc.gov.uk/employers
Named GP
From April 2015 all Practices are contractually required by NHS England to provide their patients with the name of a GP who will have overall responsibility for the care and support that the Practice provides.
This does not prevent you from seeing any GP in the practice as you currently do, nor does it mean you will always be able to see the named GP.
You do not need to take any further action, but if you wish to discuss this further with us, please do so when you next attend the surgery.
