Practice Health Champions are people who voluntarily give their time to work with the staff in their local GP Practice or surgery to find new ways to improve the services that the practice offers, and to help to meet the health needs of patients and the wider community.
Circle Dance
Every Tuesday we offer a circle dance and exercise session in the lounge area of Mytholm Meadows in Hebden Bridge. It is open to all and we are currently a mixed group both in terms of gender and ability. The idea is to bring dance, exercise and stimulation to those who may have problems with mobility or be living with some degree of dementia. As a result the activities are carried out seated in a circle and we also pass around musical instruments and visual stimulants, such as bright scarves or objects of interest. It is a lot of fun as we also share more than the occasional joke or story. It runs from 11am till 11.45am but feel free to arrive early and get a good seat. People with wheelchairs and walkers are welcome and the whole purpose is to encourage all who attend to feel connected and included.

Early is Better
An early diagnosis – whether through early reporting of symptoms or via screening – usually leads to a better outcome for the patient, with less treatment being required. (Thus saving the NHS money.)
The ‘Early is Better’ stand provides free publications that raise awareness of ‘early warning’ signs and symptoms and promote NHS Screening programmes for a variety of conditions. These include stroke, meningitis, sepsis, abdominal aortic aneurism and cancer (with particular emphasis on the four most common types of cancer – breast, prostate, bowel and lung).
Several of the PHC volunteers who run the stand have had positive outcomes from early diagnosis/screening.
The stand is currently at Valley Road and Grange Dene Medical Centres once a month. We are also aiming to take the stand out to community groups.
Hear Here
A monthly opportunity to talk about anything related to hearing loss, how to identify it, how it affects daily life and relationships. Share helpful hints. Find out what hearing aids can and can't do, how to insert them correctly and keep them in good working order. Your interests and insights will shape the conversations and directions of this initiative.

Sunny Days
Parents and children group offers a space in which any adult, together with one or more children in their care can come and socialise.
You can bring children of any age, but we ask that they are always accompanied and that you take responsibility for their safety and supervision.
It is a drop-in service from 10am-12am every Monday morning except Bank Holidays.
Most stay for the whole session and make use of the café provided by Linden Artsmill.
Teas, coffees, soft drinks and snacks are all reasonably priced.
They make up the only contribution the Artsmill gets from providing a wonderfully relaxed and welcoming space.
There are toys, books and puzzles for the children and practise health champions are on hand to offer support where they can.

Soothing Strolls
A soothing stroll is a relaxing form of exercise and is particularly recommended by Health Professionals as the best way to Health and Fitness.
The Practice Health Champions offer guided walks. An enjoyable and rewarding walk, meeting new people, experiencing the great outdoors, fresh air and gentle exercise can help to build fitness and improve health.
These low level walks are short, mainly flat with some gentle gradients. The duration can be from around 30mins up to 1hour.
If you suffer any form of ailment or are on the road to recovery from an illness we recommend you seek the advice of your Health Professional/Doctor before contemplating any form of physical activity.
Before embarking on any Walk make sure you have comfortable walking boots or stout shoes and protective clothing for all weather conditions.
Walkers should walk at a pace they find comfortable.
The walk leader should be informed if you are struggling, feel unwell or you want to leave the walk.
Please Note: Walkers take part at their own risk. Walk leaders are not responsible for any injury or Accident that might occur before, during or after the walk.
